CJR-X: What to Know & How to Prepare
August 11, 2026CMS is expanding its use of mandatory, episode-based payment models through the upcoming Comprehensive Care for Joint Replacement Expanded (CJR-X) Model, scheduled to begin in January 2028. This model shifts greater financial accountability to hospitals, with performance tied to cost and quality outcomes across defined episodes of care. CJR-X is projected to generate approximately $725 million in savings for CMS over its first five years and doesn’t currently have a defined end date.
Preparing for participation in CJR-X will require an understanding of the model’s structure, performance expectations, and potential implications for hospitals.
